On most inline 4cylinder motors above 2 litres, two counter rotating balance shafts are used rotating at double the speed of the crank shaft, usually belt driven, so adding considerable complexity.
Flat plane crank V8s have the firing order of a four cylinder motor on each bank, 1,3,4,2. great for exhaust scavenging, and are almost always under 4 litres for the same reason as they would also require balance shafts if their capacity was above 4 litres.
V8s with a conventional firing order have exhaust savaging problems, but don't have the same capacity restraints, but sometimes use a balance pipe between their 2 exhaust banks to help exhaust scavenging, a V8 flat plane crank has no exhaust scavenging problem.
Inline 6 cylinder motor pistons move in pairs, so are balanced by design.
